Before a fencer is allowed to participate at the Rockville Fencing Academy, there must be a signed waiver form on file.  Use the links below to view and print out the waiver as a .doc (Word file) or a .pdf (adobe file).

 

You may bring the signed waiver to the first class or mail / fax it in earlier.
